Rady Children’s Physician Management Services (RCPMS) offers exciting opportunities for qualified candidates to join our rapidly growing organization. RCPMS supports primary care pediatricians with 32 office locations throughout San Diego and Southern Riverside Counties.

RCPMS is seeking a friendly, energetic, and confident individual with excellent communication skills to join our team as a Patient Care Coordinator. Under the supervision of the Associate Director of Clinic Operations and Client Services, the Patient Care Coordinator will provide optimal service and support to the Population Health programs.  Working in collaboration with the RN Population Health Care Coordinator, the Patient Care Coordinator reviews various patient care dash boards, helps formulate outreach plans and participates in patient outreach activities.  The Patient Care Coordinator will also work with various CPCMG, RCHSD and Community partners to coordinate patient resources. 

Responsibilities:

  • Reviews patient dashboards to assist RN in patient outreach that fall within the specific PH program guidelines.
  • Conducts patient outreach through various communication tools; MyChart, phones or text.
  • Communicates with patients in a professional manner both verbally and in writing and is expected to respond to messages within one business day.
  • Assists patient families with care navigation including but not limited to referral information, appointment coordination and providing information and linkages to community resources.
  • Ensure familiarity and comfort level with basic chart review, patient registration, scheduling and referrals to be able to efficiently assist patient families.

Requirements:

  • Some medical referral coordination and front office experience preferred.
  • Proven superb interpersonal skills to effectively communicate and approach customers, employees, physicians and management in a tactful manner.
  • Established success in maintaining composure when confronted with fast-paced and stressful situations.
  • High school diploma or equivalent required.
