Rady Children’s Physician Management Services (RCPMS) offers exciting opportunities for qualified candidates to join our rapidly growing organization. We are a wholly owned subsidiary of Rady Children’s Hospital and Health Center with just over 500 employees. RCPMS supports primary care pediatricians with 32 office locations throughout San Diego and Southern Riverside Counties.

RCPMS is seeking a friendly, energetic, and confident individual with excellent communication skills to join our team as a full-time Pediatric Clinic Nurse Care Coordinator at our UCSD Frost Street location in Kearny Mesa. Full-time hours are Monday-Friday, 8am to 5pm.

Under the supervision of the Site Manager and in collaboration with primary care medical providers performs case management and direct patient care. This includes point of care procedures, ongoing patient and family education and coordination with outside agencies. When providing patient care and responding to telephonic health advice is responsible for independently applying the nursing process. Provides a caring relationship that facilitates health and healing.

Responsibilities:

Administer medication and immunizations per physician order
Administers medication and/or fluids via G-Tube
Provide post-procedure assessment and teaching
Provide wound care per physician order
Performs urinary catheterization per physician order
Performs G-Tube Replacement per physician order
Properly documents telephone care per RCPMS policy.
Under physician direction, notifies patient/guardian of diagnostic test results and follow-up care as indicated
Collect and analyze pertinent data and assess patient needs
Recognize urgent and emergent situations and respond calmly and professionally in stressful situations

Requirements:

Graduate of an accredited School of Nursing
Minimum of 3 years professional nursing experience, preferably in pediatrics.
2 years of NICU, PICU, or professional pediatric nursing experience taking care of medically fragile children. Skill in implementing appropriate nursing interventions.
Experience in pediatric patient teaching preferred
Current CA RN licensure.
Current BLS certification.
EMR experience required, EPIC preferred.

The reasonably expected starting hourly wage range for this position is $45.00 – $53.00 depending on qualifications including education and relevant experience.
