Rady Children’s Physician Management Services (RCPMS) offers exciting opportunities for qualified candidates to join our rapidly growing organization. RCPMS supports primary care pediatricians with 32 office locations throughout San Diego and Southern Riverside Counties.

 

Under the supervision of the Mental Health Program Supervisor and in collaboration with the Director of Behavioral and Mental Health Services- CPCMG, the Behavioral Health Care Coordinator (BH Care Coordinator) will provide optimal service and support to providers and families within the Behavioral Health, Center of Excellence program. The BH Care Coordinator will work in collaboration with the RN Behavioral Health Care Coordinator to review various patient care dashboards, formulating outreach plans and assist with patient outreach. The BH Care Coordinator will also work with various CPCMG, RCHSD and Community partners to coordinate patient resources. The BH Care Coordinator maybe assigned other administrative workflows for the Behavioral Health program.

 

Location: Kearny Mesa

 

Hours: Monday to Friday from 8:00am to 5:00pm

 

Responsibilities:

  • Review patient dashboards to assist RN in patient outreach that fall within the specific BH program guidelines.
  • Conducts patient outreach through various communication tools; MyChart, mail, phone, or text.
  • Must be able to communicate professionally both in verbal conversation and through written format.
  • Responds to messages within one (1) business day.
  • Serves as a liaison and coordinates information between various CPCMG, RCHSD or community resources.
  • Assists patient families with care navigation such as referral information, appointment coordination and community resources.
  • Familiar with basic chart review, patient registration, scheduling and referrals.

Requirements:

  • High School diploma.
  • Some medical referral coordination and front office experience preferred.
  • Thorough knowledge of NCQA credentialing standards
  • Thorough knowledge of HIPAA
  • Superb organizational skills
  • Consistent task follow-through to completion
