RCPMS supports primary care pediatricians with 32 office locations throughout San Diego and Southern Riverside Counties.

RCPMS is seeking a friendly, energetic, and confident individual with excellent communication skills to join our team as a Behavioral Health Care Coordinator RN. Under the supervision of an Operations Manager and in collaboration with Director of Behavioral and Mental Health Services- CPCMG, the Behavioral Health Care Coordinator- RN will provide optimal service and support to providers and families within the Behavioral Health, Center of Excellence program. The Behavioral Health Care Coordinator will manage the internal referral process, clinical and administrative workflows for the Behavioral Health program.

Responsibilities:

  • Appointment management to include scheduling all appointments (initial and follow-up), assignment of appropriate questionnaires and coordination with families to help facilitate completion prior to appointment.
  • Monitors provider schedule to include monitoring of cancellations and utilization of waitlist to ensure optimal program utilization.
  • Room patients, including taking vitals, effectively communicate patient concerns to provider facilitating a productive appointment for patient and provider. Assist with patient questions and review of AVS, as needed.
  • Administer medication per physician order.
  • Assist physicians with pharmacy calls that require clarification.
  • Facilitate prior authorization for medication that requires authorization as needed.
  • Provide patient education, support, and care as an extension of the physician practice.
  • Provide Telephonic Triage
  • Must be willing to travel and work between various CPCMG locations

Requirements:

  • Graduate of an accredited School of Nursing with minimum of one year professional nursing experience, preferably in pediatrics or New Grad with Clinical experience as a LVN/MA in a pediatric setting. Experience in pediatric patient teaching preferred
  • Current RN license in the state of California
  • Current BLS certificate

The reasonably expected starting hourly wage range for this position is $45-$50 depending on qualifications including education and relevant experience.
